Program / MCC Forms

Below are the forms for all of our MCC Programs. We have provided the forms in three different formats so that you can choose what works best for you. Modifications to these forms are not permitted.

Step 1 - Lender to complete Form #001 and email or fax to the Program Administrator once Applicant(s) has chosen a property:

Step 2 - Once Lender receives the Reservation Confirmation from the Program Administrator, Lender to prepare and send by fax or email the MCC Application Package to the Program Administrator. The required forms and documents are listed in Form #002.

Forms #003 and #004 are part of the MCC Application Package. They are completed by the Lender. Form #003 is executed by the Applicant(s) and the Lender and Form #004 is executed by the Seller.

Optional Forms #005 and #006 are optional forms. Form #005 is used when an Applicant has no income and does not work. Form #006 is used when an Applicant was not required to file tax returns, as applicable.

Step 3 - Once the MCC Application Package is reviewed and approved by the Program Administrator, the Lender will be emailed the MCC Commitment Letter with Forms #007 and #008. Form #007 is the checklist used to submit the MCC Closing Package to the Program Administrator after Closing. Form #008 is executed by the Applicant(s) at Closing and included in the MCC Closing Package.

Step 4 - Once the MCC Application Package is reviewed and approved by the Program Administrator, the Program Administrator will send to the Applicant(s) a welcome letter, the Mortgage Credit Certificate and Form #009. The Program Administrator completes Form #009 before sending to the Applicant(s).
